Ask Quint: July 2008

I am the Clinical Charge Nurse of a hospital based, not for profit Home Health Agency. I am trying to implement a discharge phone call policy but am struggling with what questions are more important than others. Are there any more "specific" questions that I should consider other than understanding discharge instructions, med instructions and who may have delivered excellent care? And, should the staff nurses be responsible for doing some of the follow up calls or should it always be the Clinical Charge nurse? Thanks for your help!
